Accommodation Request Form
The Request Form for current and incoming students is a quick way to get the information we need from your clinician, regardless of the disability type.
This form also includes a Housing Accommodation Request form and a Request for DR to Consult on Admissions Decisions form.
Alternative Testing Contract Questions
In order to receive accommodated testing and schedule exams in DSS, the student must first meet with their instructor and go over the answers to the Testing Contract.
Once the contract is completed, the student enters the answers into AIMS before scheduling all their exams for the term.
Use the FERPA form to consent to the release of records (to another institution, parents, etc.).
