Welcome back, SOU Raiders! 

While classes will not fully resume on campus until October 11th, 2021 the SHWC team is still here for you. 

The Student Health and Wellness Center provides access to a wide range of health services including primary medical care, mental health care, psychiatric care, preventive services and campus-wide health promotion. We are here for you health needs.

We are open for medical and mental health services. Appointments will be conducted virtually and in-person as deemed necessary. Please call 541-552-6136 to schedule an appointment.   

SHWC is not accepting walk ins at this time. If you need to schedule an appointment please call ahead. 1

COVID-19 Vaccine Requirement 

Submit your COVID-19 vaccine records or request an exemption through your "My Health Portal". SOU requires students to do the either of the following:

  1. Attest that you are fully vaccinated.
  2. File for an exemption to the vaccination requirement based on a medical, religious, or philosophical reason.
    1. Per Oregon Health Authority, students who file for an exemption must fill out either a religous or medical exemption form.Students who file exemption for a philosophical reason must upload and submit a personal statement in their "My Health Portal". There is no philosophical exemption form. These forms can be submitted on the student's "My Health Portal".

Please complete ASAP so that the team at SHWC can review, approve, and clear students for in-person, on campus activity a timely manner.

Students who opt for either a non-medical exemption or deferral will be encouraged to watch a short educational module. SOU employees, volunteers and others affiliated with the university will have the same vaccination requirements and exemption options.

The Student Health & Wellness Center will be providing the Moderna Covid vaccine, free of charge to all SOU students, SOU student employees, SOU employees, SOU Emeritus, and SOU Board of Trustees. For more information click here

 Holds on Student Accounts (please be advised it can take up to 48hrs. for changes to be reflected in our system)

For Cleared for On-Campus: This hold indicates that your status for in-person, on-campus activity has been approved, relating to COVID-19. No further action is needed.

For Not Cleared for On-Campus:This hold indicates that documents related to COVID-19 are still required. Please contact SHWC at the number below.

Fall is not just for pumpkin spice. The Student Health and Wellness encourages all students to get a flu shot. More information coming soon.

Hours and Location 

The SHWC is closed for clinical services during winter break, spring break, and from the end of summer session until the start of fall term. Part-time hours of service are provided during summer.
